A questionnaire is a tool basically structured, consisting of a more or less extensive battery of questions, many of which have default answers in which the subject is asked to choose (requests "closed"), and only a few (or no) allow a freely given response ("open" questions). This structure makes it easier for the compilation of results and, therefore, the use of the questionnaire is well suited for large sample sizes (especially representative samples) in surveys in which you intend to arrive at a quantification of their information and a statistical treatment.
As we proceed to carry out the observation?
